The role of the Health Care Coordinator is to lead the neighborhood’s team of nurses in providing excellent nursing care to the elders in an efficient and positive manner. 
 
  RESPONSIBILITIES:            
·         Reviews self-created schedules of the Clinical Support Team to assure compliance.
·         Obtains daily verbal report from the clinical support nurses (daily rounds are effectively conducted).
·         Obtains daily EMR reports (progress notes|alerts|medication exceptions).
·         Discusses elder care changes at standup effectively (falls, census, pending admits, planned discharges).
·         Discusses observations/concerns about clinical compliance with the CST nurses or care coordinator.
·         Monitors compliance of collection and submission of MDS data.
·         Reviews and assures that team is compliant with in-service requirements and program for self-development.
·         Monitors Quality Indicators and coordinates Quality Improvement.
·         Monitors effectiveness of medication reduction.
·         Monitors clinical budget and acts as a fiscal steward
·         Effectively complies with infection control policies, procedures and documentation.
·         Effectively investigates and documents falls.
·         Completes incident reporting and follow up.
·         Acts as a liaison with the Medical Director(s) and completes Monthly Reports and Quarterly QI initiatives.
·         Completes various performance reports (Wound, etc.).
·         Effectively performs performance management and hiring practices.
·         Acts as a liaison with our healthcare vendors.
·         Covers floor as appropriate.
·         Acts as a liaison with families when appropriate.
·         Other duties as assigned.
 
EXPERIENCE OR TRAINING REQUIRED
 
·         BA or BS Nursing or current Registered Nurse license in the State of Ohio.
·         2 years nurse management experience in a long-term care center.

Otterbein Skilled Nursing & Rehab Neighborhoods combine comfortable home settings with highly-trained rehabilitation staffs, creating a truly unique experience. Unlike large, institutional settings, most Otterbein Skilled Nursing & Rehab Neighborhoods are located in familiar residential areas. Each skilled nursing and rehab neighborhood is a collection of “small houses” situated within a community.
 
A total of five small houses surround a cul-de-sac, creating the neighborhood. The houses have private suites that provide a comfortable and familiar setting for elders and rehab residents. A large common room with a connected kitchen is the perfect space for socializing. No more than 10 elders and rehab residents occupy each home. Family and friends are encouraged to visit just as if they were in their own home.
 
Elders and rehab residents make choices about their daily activities, such as working on their computer, knitting/crocheting, completing puzzles or reading their favorite book, helping with household chores or spending time outdoors on the patio garden.
 
The goal of Otterbein Skilled Nursing & Rehab Neighborhoods is not only to provide superior care for elders and rehab residents, but also to help them live their life to the fullest and with dignity.

·         Proven record of securing large gifts ($100,000 - $1 million).

·         Charitable Gifts (development) experience in retirement living and long-term care communities or healthcare a plus.

  • Assists in screening and interviewing process of new organization personnel and makes recommendations for employment of individuals. Assists in the orientation of new organization personnel.

  • Assists in the formulation, revision, implementation and evaluation of policies and procedures, as well as strategic goals and objectives.
  • Participated in public relation and community activities that promote the organization’s role as an effective member of the health care delivery system.
 
EXPERIENCE OR TRAINING REQUIRED:   
  • At least five years of experience in home health
  • At least three years of management and/or large case management experience. Strong clinical skills and experience in assessing the work of clinical staff is a must.
  • OASIS/C experience is a strong plus.

OTTERBEIN Retirement Living Communities is currently seeking a Healthcare Administrator for our 250 plus bed multi-level care nursing home campus in Lebanon Ohio.
