Health Coordinator

Registered Nurse- intelligent and energetic with solid clinical leadership skills to champion excellent clinical outcomes in a creative new model of providing skilled nursing home care to 50 residents in a home care environment. 

Nurses satisfied with the current nursing home system or those who dislike change need not apply!

Experience in long term care and prior DON leadership a plus, good care planning and assessment skills essential, willingness to be part of a small efficient interdisciplinary team, must be willing to learn new ways of thinking and acting and able to lead a team to excellence!

Avalon by Otterbein, a new small house community designed to deliver skilled nursing home care to Elders in a home/person-centered setting has part-time night shift 2-3 shifts a week

full-time flexible schedule between days and evening shifts.

Special projects such as doctor day nurse, ability to pick up shifts on short notice.
