COMMUNITY: Home Health by Otterbein

RESPONSIBILITIES:

Provide direct physical therapy treatments in accordance with physician’s plan of care.

Treats patients to relieve pain, develop or restore function, and maintain maximum performance.

Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.

Utilizes physical agents, including, heat., cold, water, light, electricity, ultrasound, and massage.

Reports to Executive Director and and Organizational personnel the patients reaction to treatment and any changes in the patient’s condition.

Maintains necessary records, clinical notes, and conference notes, which will be incorporated into the patient’s clinical record in prescribed time frames.

Participates in Organizational Home Health Team Members meetings and Organizational In service Educational Programs

EXPERIENCE OR TRAINING REQUIRED

Two Years PA experience

Home Health experience a plus

PT Certificate approved by American Physical Therapy Association

Travel Required within Warren, Clinton Counties and the surrounding counties. Reliable transportation and good driving record is required
